For starters, you need to have a national ID. If you don’t have one, a copy of your birth certificate authenticated by the National Statistics Office (NSO) will do. You can easily acquire this online.
You also need to be at least 17 years old, but you must present an original copy of your parent’s consent or affidavit of a person having custody/care supervision over you. Submit the document with a photocopy of one valid, government-issued ID with a photo and signature of the affiant. We can make you a template of this if you’d like.
Married women have to present their marriage contracts to the LTO if they will use their spouses’ names in their licenses.
